Hair toxicological Analyses–Drug / pharmaceutical substances and alcohol testing

General Information

Hair toxicological analyses are used to determine if someone has used drugs or pharmaceutical substances in earlier periods.

Scalp hairScalp hair grows at an average rate of about 1 to 1.5 cm per month. Each 1 to 1.5 cm of hair represents the history of substance use for 1 month. For example, a 12-cm long lock of hair contains the history of substance use of approximately 12 months.

→ Hair toxicological analysis allows investigation of consumption in earlier periods as well as determination of the user profile: occasional user, daily user, etc., while it is possible to precisely determine the amount of grams consumed per week or month for most drug and pharmaceutical substances.

Alternative hair sampling sites

Alternative hair sampling sites: Hair samples can also be collected from other areas of the body, such as the axilla, genitals, chest, hands, beard etc., also providing information about earlier use; however, it is not possible to precisely determine the exact time period of substance consumption because hair growth rate in these areas is not specific.

Detection Window: from 7 days to years (depends on hair length)
For any substance detection, about 7 days are required for its decomposition and transfer to the hair through blood circulation

Types of hair analyses: Full and sectional analysis

Sectional analysis
Sectional scalp hair analysis is the most detailed method for detecting drugs in hair. Cutting hair into 1 cm segments allows determination of the use per month and provides a detailed consumption history profile. Furthermore, sectional analysis is useful in cases where the trend of use has to be determined, e.g. the increase or decrease in the usage of a drug over time, or to confirm its discontinuation.

Full analysis
Full scalp hair analysis provides an overall image for the use of a drug for the period under study. Every analysis covers maximum a 3-month period and depending on the available hair length the use can be determined for the last 3, 6, 9, 12 months and so on.

Sample collection: Collection of 15 to 200 hairs (depending on the type of analysis to be performed) at least 1 cm long.

Hair alcohol testing is performed by detecting the biomarker ethyl glucuronide (EtG) a metabolite of ethanol. The test requires at least 200 hairs with a minimum length of 3 cm. The test can be performed with hair from other parts of the body (arms, chest, genitals) but the accuracy of the test is reduced.

→ The results will determine if a person presents chronic and excessive alcohol consumption, if he is an occasional social drinker, or if he/she abstains completely from alcohol.

→ A typical dose is equivalent to 14g of pure alcohol, which corresponds to 350mL of beer (5% alcohol), 150mL of wine (12% alcohol) or 45mL of spirit (vodka, whiskey, gin, tequila, with 40% alcohol).
